Passport Acceptance Facility
The Clerk of Court is an officially designated acceptance facility for the U.S. Department of State.
We accept applications for new passports (Form DS-11). This includes:
- First-time applicants.
- Minors under age 16.
- Applicants whose previous passport was issued over 15 years ago.
- Applicants whose previous passport was lost, stolen, or damaged.
Renewals (Form DS-82)
Adult renewals are generally mailed by the applicant and are not processed in our office. You can pick up a renewal form here or download it from travel.state.gov.
Checklist: What to Bring
Federal regulations are strict. Failure to bring any of the following may result in us being unable to process your application.
1. Form DS-11
Completed in black ink only. Do not sign the application until instructed by the Clerk.
2. Citizenship
Original Birth Certificate (with raised seal, listing parents) or Naturalization Certificate.
(Originals are mailed with application and returned later.)
3. Photo ID
Valid Driver's License, Military ID, or Government ID. We will need to make a photocopy of the front and back.
4. Passport Photo
One 2x2 inch color photo taken within the last 6 months. Glasses must be removed.
We take photos on-site for $10.00.
Federal regulations require fees to be split. You must be prepared to make two separate payments.
Payment #1: The Application Fee
This fee goes to the federal government for processing.

Routine Service
6-8 Weeks
Standard processing time via mail.
Expedited Service
2-3 Weeks
Requires additional $60 fee.

Special Rules for Minors
Federal law mandates strict requirements for children under age 16 to prevent international parental abduction.
- Both parents must appear in person with the child.
- If one parent cannot appear, a notarized Form DS-3053 (Statement of Consent) is required from the absent parent.
- You must submit the child's birth certificate even if they have had a previous passport.
